Our services require an active membership and include in-person consultations, diagnostic testing, and personalized care planning.

Enrollment and Fees

Membership requires completion of enrollment forms and payment of applicable fees. Membership fees are billed according to your selected membership tier and payment schedule.

Payment Terms

All fees are due at the time of service or according to your membership agreement. We accept major credit cards and electronic payments. You are responsible for maintaining current payment information.

Cancellation Policy

You may cancel your membership with 30 days' written notice. Cancellation requests must be submitted via email to info@emeraldpeak.health. No refunds will be issued for partial months.

Refund Policy

Membership fees are non-refundable except as required by law. Unused services do not carry over after membership termination.

As a user of our Services, you agree to:

• Provide accurate, current, and complete information during enrollment and use of services
• Maintain the confidentiality of your account credentials
• Use the Services only for lawful purposes and in compliance with all applicable laws
• Arrive on time for scheduled appointments or provide at least 24 hours' notice for cancellations
