Month 0
Comprehensive Onboarding
Establish the medical baseline, define priorities, and create the initial Emerald Peak Action Plan.
Foundational laboratory testing should generally be completed beforehand when feasible.
The initial Performance assessment is expected to occur during the early membership period through Emerald Peak or a designated Performance partner.
Month 4
Progress Review
Review progress, interval medical issues, relevant measurements, medications, and the Action Plan.
At the planned full-location model, an included Wellness Room session is available during the same calendar month.
Month 6
Performance Reassessment
A Performance reassessment is planned during the first membership year when partner economics and availability allow.
The objective is to determine whether physical capacity is moving in the intended direction and whether the strategy should change.
Month 8
Progress Review
Return to the goals, evaluate what has changed, and set the priorities for the next phase.
At the planned full-location model, another included Wellness Room session is available during the same calendar month.
Month 12
Annual Summit
Review the year, repeat appropriate measurements, reassess risk, and set the strategy for the next membership year.
Month 12 also serves as the starting point for the next annual cycle.
At the planned full-location model, the corresponding Wellness Room session is the third included session of the membership year.

Emerald Peak is built around longitudinal care rather than episodic transactions, so Medicine membership uses an annual commitment. Payment, renewal, cancellation, and termination terms are governed by the Membership Agreement.

Measure Physical Capacity.
Emerald Peak strongly recommends objective Performance assessment. The initial assessment establishes a physical baseline, and with your authorization the findings inform your Action Plan. A reassessment is planned during the first membership year when partner arrangements and availability allow.
If ongoing Performance services are provided by an independent partner, those services may be separately contracted and billed.
The Psychological Side of Health.
Stress, behavior, focus, sleep, recovery, and executive performance influence whether a health strategy succeeds. When Mindset expertise would add value, Emerald Peak can connect you with appropriate professionals and, with authorization, incorporate their recommendations into your Action Plan.
Mindset sessions are not included in the Medicine membership at launch. Mindset professionals may be independent providers who contract and bill separately.
Recovery, Integrated.
The planned Emerald Peak location will include a private Wellness Room that brings recovery into the member experience.
Membership includes three Wellness Room sessions per 12-month membership year, aligned with the recurring Medicine touchpoints. Additional member-only access is planned.
Planned modalities include

- Zero-gravity meditation and recovery chair
Active Medicine members receive one included session approximately every four months, usable anytime during the same calendar month as the corresponding Medicine visit.
The Month 12 session is the third included session of that membership year and does not create a duplicate benefit in the next annual cycle.
Unused included sessions do not roll over, have no cash value, and are nontransferable.
Additional access will be available through member-only sessions and monthly access options.
Membership Is Not Insurance.
Membership pays for the services and relationship included in the membership. It is not health insurance and does not replace appropriate insurance or other financial protection for hospitalization, emergency care, specialist care, imaging, procedures, medications, and other major medical expenses. Emerald Peak does not guarantee insurance reimbursement for membership fees or outside services.
The practice is unable to accept patients who have Medicare or Medicare Advantage plans as members at this time due to federal regulations governing Direct Primary Care practices.
